The federal HealthCare.gov website now provides the individual health insurance application denial rate for each insurance company (approximately 299) listed on the website.  The website also gives the percentage of applicants charged higher premiums based on health status.

Currently, the site lists approximately 8,500 individual and family health insurance plans.
